How long does the assessment last?
The process may take anywhere from 1 hour to 1 ½ hours.
What do I need to bring to the assessment?
Please arrive with a list of current medications and your insurance card.
Who may attend the assessment?
We meet with you individually but you may bring someone to support you.
Am I obligated to start the program after the assessment is completed?
Do I really need to fill out all of this paperwork?
Yes. In order to protect your confidentiality and explain your rights, it is important you fill out all of the paperwork. Also, we must get an accurate health history so we can work with your insurance company and understand your chemical dependency needs. Finally, we want to give you as accurate an estimate as we can of any fees you might incur.
What insurances do you take & do they all pay for your program?
Eleventh Hour is contracted with most insurance companies. Unfortunately, we do not accept Medicare, Medi-Cal or Tricare. All insurances are different & you must meet their criteria for any level of care. This is why the assessment is so important, so be as honest as you can.
What if I do not have insurance?
The Eleventh Hour accepts all major credit cards for payment. Also, we can work on a payment plan for you if necessary.
What days and hours does the Day Treatment Program meet?
Our Day Treatment Program meets as follows:
- Monday, Tuesday, Wednesday, Friday 9:00am to 4:00pm.
- Thursday 9:00am-12:00pm then return at 6:00pm for the family group which meets until 9:00pm
What are the days and hours of your Intensive Outpatient Program?
Our Intensive Outpatient Program has two tracks available to you. The evening program meets Monday, Wednesday and Thursday from 6:00pm to 9:00pm and the family group is Wednesday evening. The day intensive outpatient program meets Monday and Thursday from 9:00am to noon. The family group is Tuesday evening from 6:00pm to 9:00pm.
Does my family have to be involved?
Eleventh Hour encourages family support in our family group.
How much is the Aftercare Program?
If you successfully complete a phase of treatment at the Eleventh Hour, 6 months of Aftercare is included free of charge. This is to be started immediately after discharge from your Day Treatment and/or Intensive Outpatient Program.
Am I required to attend the Aftercare Program?
If you do not begin Aftercare immediately after completing your Day Treatment or Intensive Outpatient Program, it may not be available to you at a later date.
What happens if I am unavailable to attend one of the groups due to a previous commitment?
If you have a scheduling conflict, please discuss this with your counselor.
